Welcome to the Medicine Request System for Barangay 35 ("System", "Service"). These Terms and Conditions ("Terms") govern your use of our system and services. By creating an account or using the Service, you agree to be bound by these Terms.
1. Acceptance of Terms
By accessing and using this System, you accept and agree to be bound by the terms and provision of this agreement. In addition, when using this System's particular services, you shall be subject to any posted guidelines or rules applicable to such services. Any participation in this service will constitute acceptance of this agreement.
2. User Accounts
To use the Service, you must register for an account. You agree to:
- Provide true, accurate, current, and complete information about yourself as prompted by the registration form.
- Maintain and promptly update your registration data to keep it true, accurate, current, and complete.
- You are responsible for maintaining the confidentiality of your password and account and are fully responsible for all activities that occur under your password or account.
- You must provide a valid Barangay ID and/or other requested documents to verify your identity as a resident of Barangay 35.
3. Medicine Requests
You agree that all medicine requests are subject to approval by authorized Barangay Health Office personnel.
- Prescriptions: For medicines that require a prescription, you must upload a clear, valid, and unexpired prescription from a licensed physician. Failure to do so will result in the rejection of your request.
- Approval: The approval of your request is based on the validity of your prescription and the availability of medicine stocks.
- No Guarantee: We do not guarantee that all requested medicines will be available or approved.
- Pickup: You must present your valid Barangay ID and the original prescription (if applicable) upon pickup.
4. Prohibited Conduct
You agree not to use the Service to:
- Submit false, misleading, or fraudulent information, including fake prescriptions.
- Attempt to request quantities of medicine deemed excessive by health officials.
- Create multiple accounts for a single individual.
- Interfere with or disrupt the security or integrity of the System.
Violation of these terms may result in the suspension or termination of your account and future access to the Service.
5. Disclaimers
The Medicine Request System is provided on an "AS IS" and "AS AVAILABLE" basis. The Barangay 35 Health Office disclaims all warranties of any kind, whether express or implied.
This service is an administrative tool to facilitate medicine requests. It does not provide medical advice. Always consult with a qualified healthcare professional for any medical questions or concerns.
6. Limitation of Liability
In no event shall the Barangay 35 Health Office or its staff be liable for any indirect, incidental, or consequential damages arising out of or in connection with your use of the Service.
7. Changes to Terms
We reserve the right, at our sole discretion, to modify or replace these Terms at any time. We will provide notice of any changes by posting the new Terms on this page. Your continued use of the Service after any such changes constitutes your acceptance of the new Terms.
